PrognoCIS patient portal is a secure web application that allows you to schedule appointments, access your test results and medical records, pay bills and communicate with your provider.
If you are a new patient or established patient who has never used the portal before please start with step 1.
If you have used our portal before please skip to step 3.
If you have forgotten your username or password please follow the directions at the bottom of this page under Forgot Your Username or Password.

For New Patients and Established Patients accessing portal for the first time:
1. Please call North Scottsdale Women's Care and schedule your appointment, at this time we will register you for the portal giving you a username and temporary password.
2. You will then receive an email inviting you to sign up for your patient portal with the following link:
https://nswc.prognocis.com/prognocis/nswcClinicIndex.html
3. Click on the link to change your password which must be 8 to 10 characters long and include at least one alpha and at least one numeral.

Type In:
1. User ID
2. Temporary Password
3. Create Password
4. Confirm Password

Return to the PrognoCIS EMR Portal Login
- Enter your User ID and the password you just created.
- Click "Log In" to access your account.

COMPLETING YOUR FORMS
If you are a New Patient or an Existing Patient – returning for your Annual Exam, you will need to fill out an electronic questionnaire as well as update your demographic information.
After logging in, you should see the patient dashboard. If you are not seeing the patient dashboard, please click on the skip button until you see the dashboard. Next, click on the menu button on the top right of this page. The "menu" button takes the form of an icon that consists of three parallel horizontal lines (displayed as ≡). Please click on patient forms and complete patient forms and legal documents. Please complete the following forms:
- Acknowledgement of Receipt of Privacy Notice
- Authorization for Use or Disclose My Health Information
- Well Woman Examinations
- GYN FORM-Detailed
Click on each form to open it. Complete the required fields and provide accurate information. Be sure to save or submit each form as you finish.
You may fill these out prior to your visit. If you chose not to fill these out prior to your visit, we kindly ask that you arrive 30 minutes prior to your scheduled appointment to ensure that you have an adequate amount of time to complete your questionnaire.
Forgot Your Username or Password?
If you ever forget your login details, including your user ID, follow these simple steps:
- Click on 'Forgot password.'
- Next, select 'Forgot login details.'
- Enter your basic details and click 'Submit.'
- You'll receive an OTP (One-Time Password) on your registered email address and cellphone.
- Enter the OTP when prompted.
- You'll now have access to your user ID and can update your login information as needed.
